The applicants should provide:

1. Declaration of Eligibility
2. Declaration of Conformity
3. A copy of the most recent accreditation certificate relating to the program issued by the relevant authority
4. An application dossier/form comprising:
   1. Cover form: Basic applicant information
   2. Program description
   3. Program curriculum
   4. Program infrastructure and resources
   5. Program statistics
   6. Program self-assessment
   7. Other documentation
